A snack is provided.  If your student(s) has/have allergies, please provide a snack for the student(s) and tell their teacher(s).

All students must be potty trained. No pull-ups will be allowed.

I (we), the parent(s) or guardian(s) of the minor child(ren) listed in this form, do hereby authorize adult volunteers of St. Peter Lutheran Church and School as agent(s) on my/our behalf, to consent to any medical or surgical care deemed advisable by any accredited physician or surgeon in an approved emergency clinic or hospital.  I further release from any liability any of its ministries or leaders in the event of an accident en route, during and returning from the above mentioned event. This agreement does not apply to claims for intentional misconduct or gross negligence.
